外周T细胞淋巴瘤组织中GCLC和GCLM的表达及其临床意义

摘    要:目的:探讨外周T细胞淋巴瘤(PTCL)组织中谷氨酸-半胱氨酸连接酶催化亚基(GCLC)和谷氨酸-半胱氨酸连接酶调节亚基(GCLM)的表达及其临床意义。方法:采用免疫组织化学法检测40例PTCL患者组织标本及20例淋巴结反应性增生组织中GCLC和GCLM的表达情况,分析其表达与PTCL患者临床病理特征的关系。结果:GCLC在PTCL组织中的阳性表达率为32.5%(13/40),在淋巴结反应性增生组织中未见明显表达,差异显著(P<0.05)。PTCL组织中GCLM的阳性表达率为35.0%(14/40),高于淋巴结反应性增生组织的10.0%,差异显著(P<0.05)。GCLC和GCLM的表达与PTCL患者的Ann-Arbor临床分期、B症状、国际预后指数(IPI)密切相关(P<0.05),但与患者的年龄、性别、乳酸脱氢酶(LDH)水平、骨髓侵犯均无关(P>0.05),且分期越晚(Ⅲ+Ⅳ期)、IPI(3~5分)越高者,GCLC和GCLM的阳性表达率越高。Spearman等级相关分析显示,PTCL组织中GCLC蛋白与GCLM蛋白的表达呈正相关。结论:GCLC和GCLM在PTCL组织中高表达,可能参与肿瘤的发生和发展,有望为PTCL的病因学研究及治疗提供理论依据。

Abstract:Objective:To determine the expression and prognostic value of glutamate-cysteine ligase catalytic subunit(GCLC)and glutamate-cysteine ligase modifier subunit(GCLM)in peripheral T-cell lymphoma(PTCL)tissues.Methods:The expression of GCLC and GCLM was assessed in 40 cases of PTCL patients and 20 patients with lymph node reactive hyperplasia tissues by using immunohistochemistry(IHC).The correlation of GCLC and GCLM expression with the clinicopathological characteristics of patients was analyzed.Results:The positive rates of GCLC in PTCL tissues were 32.5%(13/40),while GCLC protein was not obviously expressed in lymph node reactive hyperplasia tissues(P<0.05).The positive rate of GCLM protein in PTCL tissues was 35.0%(14/40),which was significantly higher than that in the lymph node reactive hyperplasia tissues(10.0%)(P<0.05).The expression of GCLC and GCLM was correlated with Ann-Arbor clinical stage,B symptoms and international prognostic index(IPI),but not associated with age,gender,bone marrow metastasis or LDH level(P<0.05).The advanced stage(Ⅲ+Ⅳ)and higher IPI(3~5)were associated with higher positive rates of GCLC and GCLM.Spearman rank correlation analysis showed that the expression of GCLC protein was positively correlated with the expression of GCLM protein in PTCL.Conclusion:The high expression of GCLC and GCLM may be involved in the carcinogenesis and progression of PTCL,which may provide theoretical basis for the etiology and treatment studies of PTCL.
