新型冠状病毒感染疫情下放射性肺炎的诊治

2019冠状病毒病（corona virus disease 2019,COVID-19）传染性极强，以肺部及免疫系统损害为主。重症患者可迅速进展至急性呼吸窘迫综合征、脓毒症休克、难以纠正的代谢性酸中毒和出凝血功能障碍及多器官功能衰竭等。放射性肺损伤是胸部肿瘤放射治疗后常见的并发症，发生率高，3级以上的放射性肺炎可迅速进展至呼吸衰竭，死亡率高达50%以上。肿瘤患者抵抗力差，更容易合并肺部感染。目前在新型冠状病毒疫情下，对胸部肿瘤放射治疗引起的放射性炎症和新型冠状病毒感染肺炎准确识别和处理尤为重要。本文简要的阐述了新型冠状病毒肺炎与放射性肺炎的诊治策略，旨在为相关诊疗提供参考。

Diagnosis and treatment of radiation pneumonitis under influence of novel coronavirus outbreak

Coronavirus disease 2019(COVID-19)is highly infectious,mainly involving damages to the lungs and the immune system.Severe patients can quickly progress to acute respiratory distress syndrome,septic shock,refractory metabolic acidosis,coagulation dysfunction and multi-organ failure.Radiation-induced lung injury is a common complication after radiotherapy of chest tumor,with a high incidence.Radiation pneumonitis of grade 3 or above can rapidly progress to respiratory failure,with a mortality of more than 50%.Cancer patients have poor resistance and are more likely to develop lung infections.At present,under the novel coronavirus outbreak,it is particularly important to accurately identify and to manage the radiation inflammation induced by radiotherapy of chest tumor and the pneumonia caused by novel coronavirus infection.In this paper,the diagnosis and treatment strategies of the novel coronavirus disease and radiation pneumonitis are described in brief.
