HPV与血清肿瘤标志物检测在宫颈癌诊断中的效能

目的:观察人乳头瘤病毒(HPV)与血清肿瘤标志物检测在宫颈癌诊断中的效能。方法:选取120例疑似宫颈癌患者作为研究对象,其中经手术病理确诊为宫颈癌54例设为宫颈癌组,宫颈良性肿瘤66例设为良性肿瘤组。另选取同期70例健康女性为对照组。比较三组HPV阳性检出率、血清肿瘤标志物糖类抗原(CA)125、CA153和CA199水平,以术后病理检查结果为“金标准”,比较HPV及CA125、CA153、CA199单独检测与联合检测在宫颈癌诊断中的效能。结果:宫颈癌组HPV16、HPV18单独检测与联合检测阳性率均高于良性肿瘤组和对照组,差异有统计学意义(P<0.05);宫颈癌组CA125、CA153、CA199水平均高于良性肿瘤组和对照组,差异有统计学意义(P<0.05);HPV与血清肿瘤标志物联合检测的灵敏度、准确度均高于单独检测,特异度低于CA153、CA199单独检测,高于HPV、CA125单独检测,差异有统计学意义(P<0.05)。结论:HPV与CA125、CA153、CA199联合检测可提高宫颈癌诊断的准确度和灵敏度。

Efficacies of HPV and serum tumor markers in diagnosis of cervical cancer

Objective:To explore efficacies of human papillomavirus(HPV)and serum tumor markers in diagnosis of cervical cancer.Methods:120 patients with suspected cervical cancer were selected as the research objects.Among them,54 cases of cervical cancer diagnosed by surgery and pathology were set as the cervical cancer group,and 66 cases of cervical benign tumors were set as the benign tumor group.In addition,70 healthy women in the same period were selected as the control group.The HPV positive detection rate and the levels of the serum tumor marker carbohydrate antigen 125(CA125),CA153 and CA199 were compared among the three groups.Taking the results of postoperative pathological examinations as the“gold standard”,the efficacies of the separate detection of HPV,CA125,CA153,and CA199 as well as the combined detection in the diagnosis of cervical cancer were compared.Results:The positive rates of HPV16 and HPV18 separate detections and combined detection in the cervical cancer group were higher than those in the benign tumor group and the control group,and the differences were statistically significant(P<0.05).The levels of CA125,CA153,and CA199 in the cervical cancer group were higher than those in the benign tumor group and the control group,and the differences were statistically significant(P<0.05).The sensitivity and accuracy of the combined detection of HPV and serum tumor markers were higher than that of the separate detection;the specificity was lower than those of CA153 and CA199 separate detections,and higher than those of HPV and CA125 separate detections;and the differences were statistically significant(P<0.05).Conclusions:The combined detection of HPV and CA125,CA153,and CA199 can improve the accuracy and sensitivity of cervical cancer diagnosis.
