Evaluation on the Diagnostic value of HBA2 for Thalassemia by Sebia Caplillary Electrophresis Apparatus

Objective To evaluate the diagnostic value of HBA2 for thalassemia by Sebia Caplillary Electrophresis Apparatus.Methods 2892 cases from prenatal screen pregnant woman with suspected anemia were studied,HBA2 was tested by Sebia Caplillary Electrophresis Apparatus,Gene diagnosis of α,β-thalassermia were by Gap-PCR and PCR-RDB methods,respectively.They were 588 α-thalassemia carriers,398β-thalassemia carriers and 1906 normal controls by gene diagnosis as a gold standard method.Results AUCROC of HBA2 for diagnosis α,β-thalassemia were 0.822,0.988,respectively.When ≥ 3.45% was taken as the cutoff value of HbA2 for diagnosis of β-thal assemia carriers,the evaluation indexes values were shown as follows: sensibility(Sen) 96.00%,Specificity(Spe) 98.70%,positive predictive values(+PV) 97.70%,negative predictive values(-PV) 1.7%,positive likelihood ratios(+LR) 99.39,negative likelihood ratios(-LR) 0.04,Accuracy(Acc) 98.70%,Youden index 0.95,If ≤ 2.65% was taken as the cutoff value of HbA2 for diagnosis of α-thal assemia carriers,the evaluation indexes values were shown as follows: Sen82.20%,Spe56.00%,+PV37.79%,-PV6.86%,+LR1.94,-LR0.23,Acc 63.00%.3.The diagnostic value of HBA2 For α-thalassemia after HBA2 combined with MCV was increased as below.Sen 98.10% in parallelly,Spe 96.20% in sequence.Conclusion The diagnostic value of HBA2 for β-thalassemia by Sebia Caplillary Electrophresis Apparatus was much better than for α-thalassemia.The diagnostic value of HBA2 for α-thalassemia was improved obviously by HBA2 combined with MCV.
