| 摘 要： | 目的探讨不同术后镇痛方式对下肢骨折行内固定患者术后体温变化的影响。方法选择150例下肢骨折行内固定术患者,根据术后镇痛情况分为硬膜外镇痛组(E组)、静脉镇痛组(I组)及prn肌注镇痛组(C组),每组50例。观察三组术后3d的体温变化和寒战发生情况。结果手术前三组体温均在37.5℃以下。术后E组正常体温40例,占80%,C组正常体温25例,占50%,I组正常体温20例,占40%。E组体温增高人数明显低于CI、组(P<0.05)。术后第1d C、I组平均体温高于E组(P<0.05)。术后E组寒战发生率和寒战程度高于I、C组(P<0.05)。结论术后硬膜外镇痛可避免下肢骨折行内固定患者术后体温增高,减轻患者不良应激,但术后需加强患者的体温监测及护理,防止低体温,而静脉镇痛使患者术后体温增高。
