直肠癌P16蛋白表达的检测及其意义

目的探讨P16蛋白在直肠癌组织中的表达及其意义。方法用SP法进行石蜡切片免疫组化染色，检测57例直肠癌组织、46例癌旁组织及20例正常直肠组织中P16蛋白的表达。结果癌组织与癌旁组织及正常组织中P16蛋白的阳性表达率分别为78.9%、89.1%、100%，前二者间及后二者间阳性率无显著性差异，但前者与后者却有显著性差异；低分化腺癌阳性表达率为54.5%，高-中分化腺癌为86.9%，二者间有显著性差异；P16蛋白的阳性表达部位在正常组织和癌旁组织中为核型，在癌组织中为核浆型、浆型，有显著性差异；随着直肠癌Dukes分期的下降，P16蛋白在癌组织中的缺失率明显增高。结论直肠癌组织中P16蛋白的阳性表达与肿瘤的发生和分化程度及临床分期有一定的关系；P16蛋白的表达部位对良性及恶性病变可能具有鉴别意义；检测P16蛋白对判断预后有重要意义。

Significance of P16protein expression in rectum cancer

Objective To study the expression of P16 protein in rectum cancer and the significance.Methods The expression of P16 protein in 57 cases of rectum cancer, 46 cases of adjacent tissue and 20 cases of normal rectum tissue was tested by immunohistochemical SP method.Results The expression rates in the three groups were 78.9%, 89.1% and 100% respectively. There were no significance differences between normal tissue and adjacent tissue, and between adjacent tissue and cancer. But a significance difference was revealed between the cancer and normal tissues. The expression of P16 protein in low differentiated adenocarcinoma was 54.5% and 86.9% in high-middle differentiated adenocarcinoma. The difference was significance. The P16 protein was found in nuclei and plasma in cancer but nucleus in normal and adjacent tissues. The loss of P16 seemed to be related with the Duke's degree. Conclusion The expression of P16 protein in rectum cancer is related to their histological differentiation and clinical stage. The expression location of P16 protein could be a characteristic for identifying benign and malignant tissue. It would be helpful for prognosis of rectum cancer patients.
