端粒酶活性检测在良恶性胸、腹水鉴别诊断中的价值

目的 探讨脱落细胞中端粒酶活性在良恶性胸、腹水鉴别诊断中的价值.方法 应用端粒酶TRAP-PCRELISA法分别检测70例胸腹水患者脱落细胞中的端粒酶活性,并与细胞学进行对比分析.结果 恶性胸腹水中的端粒酶活性水平明显高于良性胸腹水,在良性腹水中端粒酶活性的阳性率为10%,明显低于恶性腹水中端粒酶活性的阳性率85%,同时恶性腹水中端粒酶活性的阳性率同脱落细胞病理学检查相比较,亦具有显著性差异(P＜0.01).在良性胸水中端粒酶活性的阳性率为10%,亦明显低于恶性胸水端粒酶活性的阳性率85%,但同脱落细胞病理学检查的阳性率相比,无显著性差异(P＞0.05).结论 脱落细胞中端粒酶活性检测可作为良恶性胸、腹水的鉴别诊断标志.

The differential diagnostic value of telomerase activity in benign and malignant ascites and pleural fluid

Objective To investigate the changes of telomerase activity in benign and malignant ascites and pleural fluid.Methods The technique of telomerase TRAP-PCR-ELISA was employed to detect telomerase activity in ascites and pleural fluids from 70 patients with benign or malignant ascites and pleural fluids.The cytological diagnosis was compared.Results Telomerase activity in malignant ascites and pleural fluid were significantly higher than that in benign group.Positive rate of telomerase activity detected in malignant ascites(85%)was remarkably higher than that in benign group(10%);Compared with the cytological diagnosis,telomerase test had higher sensitivity(85%),specificity(90%)and stability.Positive rate of telomerase activity detected in malignant pleural fluid(85%)was remarkably higher than that in benign group(10%),but compared with the cytological diagnosis,the results had no difference.Conclusion Telomerase activity may be an useful and sensitive marker in the differential diagnosis of benign and malignant ascites and pleural fluid.
