实时三维超声评价单心室的节段功能和同步运动的研究

摘    要:目的:探讨实时三维超声心动图评价功能性单心室患儿心功能和同步性的应用价值。方法选取功能性单心室患儿20例,正常儿童20例,Q‐lab软件分析并计算比较主心腔的舒张末期容积、收缩末期容积、射血分数、节段位移和圆心位移值以及各项同步性各项参数。结果①单心室患儿舒张末期容积及收缩末期容积比正常儿童明显增大,而射血分数却显著降低;②单心室患儿各节段的位移均低于正常儿童;③单心室患儿 Tmsv16‐SD、Tmsv16‐SD%、Tmsv16‐Dif、Tmsv16‐Dif%、Tmsv12‐SD、Tmsv12‐SD%,Tmsv12‐Dif、Tmsv12‐Dif%、Tmsv6‐SD、Tmsv6‐SD%、Tmsv6‐Dif、Tmsv6‐Dif%较正常儿童有明显延长;④同步性指标和心功能相关性较好,Tmsv16‐SD%与 EF相关性最好( r =‐0.774,P <0.001)。结论 Tmsv16‐SD%与EF相关性最好,是反映心肌运动同步性的理想指标。

Abstract:Objective To explore the value of three‐dimensional echocardiography for assessment of ventricular function and synchronicity in functional single ventricle (FSV) .Methods 3DE data were acquired in 20 patients with FSV and 20 age‐matched normal children ,then ,end‐diastolic volume (EDV) ,end‐systolic volume (ESV) ,ejection fraction (EF) ,ex‐cursion ,ball excursion and indexes of ventricular synchronicity were compared by Q‐lab software .Results ① EDV and ESV were significantly higher and EF were significant significantly lower in FSV group ;②Excursion of various segments was lower in FSV group ; ③ The values of Tmsv16‐SD , Tmsv16‐SD% , Tmsv16‐Dif , Tmsv16‐Dif% , Tmsv12‐SD , Tmsv12‐SD% ,Tmsv12‐Dif , Tmsv12‐Dif% , Tmsv6‐SD% , Tmsv6‐SD% , Tmsv6‐Dif , Tmsv6‐Dif% were significantly prolonged in FSV ;④Most indexes of LV synchronicity had good correlation with ventricular function ,especially Tmsv16‐SD% ( r =‐0 .774 ,P<0.001) .Conclusion Tmsv16‐SD% ,best correlated with ventricular function can provide an opti‐mal index in assessing ventricular synchronicity .
