摘    要:[目的]研究糖尿病肾病(diabetic nephropathy,DN)患者血浆脑利钠肽(BNP)水平及其与糖尿病肾病的关系。[方法]随机选择2型糖尿病患者106例,按尿微量白蛋白排泄率(UAER)分为大量蛋白尿组(UAER〉200μg/min36例),微量白蛋白尿组(UAER 20-200μg/min 38例),非白蛋白尿组(UAER〈20μg/min 32例),健康成人30例作为对照组。应用电化学发光法测定各组BNP水平。[结果]微量白蛋白尿组、大量白蛋白尿组血浆BNP水平明显高于非白蛋白尿组(分别为(192.1±72.5)pg/mL、(429.9±165/3)pg/mL、(101.3±46.7)pg/mL,P〈0.01),后者高于正常对照组,(57.6±43.4)pg/mL,P〈0.01,且与UAER呈明显正相关(r=0.83.P〈0.01)。[结论]糖尿病肾病患者血浆BNP水平明显升高,可能在糖尿病肾病发生、发展过程中发挥重要作用。

Abstract:[ Objective] To investigate the relationship between plasma brain natriuretic peptide level in diabetic patients and diabetic nephropathy, and to evaluate its clinical significance simutanously. [ Methods ] One hundred and six patients with type 2 diabetic mellitus were enrolled at random. According to urinary albumin execration rate ( UAER), 106 patients were divided into three groups : macroalbuminuric group ( UAER 〉 200 μg/min), microalbuminuric group ( 20 μg/min 〈 UAER 〈 200 txg/min) and nonalbuminuric group( UAER 〈20 μg/min). The plasma levels of BNP were measured with electrochemiluminescence immunoassay ( ECLIA ). [ Result ] The plasma BNP levels of macroalbuminuric group and microalbuminuric group were significantly higher than those of nonalbuminuric group ( 192.1±72.5 ) pg/mL, (429.9± 165.3 ) pg/mL, ( 101.3 ±46.7) pg/mL,P 〈0.01. The blood BNP concentration of nonalbuminuric group increase significantly compared with control group ( 101.3±46.7 ) pg/mL, (57.6±43.4 ) pg/mL, P 〈 0.01. The plasma levels of BNP were positively and significantly correlated with UAER in diabetic nephropathy. [ Conclusion] The plasma levels of BNP were significantly increased in patients with diabetic nephropathy, which probably play an important role in the ocurrence and development of this disease.
