髓母细胞瘤中血管内皮生长因子及其受体的表达和临床意义

摘    要:目的探讨血管内皮生长因子(vascular endothelial growth factor,VEGF)及其受体激酶插入嵌合受体(kinase insert domain-containning receptor,KDR)在髓母细胞瘤中的表达情况及它们与肿瘤微血管密度(microvascular density,MVD)以及患者预后的关系。方法通过免疫组化方法测定VEGF、KDR在45例髓母细胞瘤和10例正常小脑组织中的表达,并结合临床资料,对病人生存时间与VEGF、KDR表达阳性率、肿瘤MVD之间关系进行分析。结果①正常小脑组织标本极少或不表达VEGF、KDR,而髓母细胞瘤标本多为不同程度的阳性表达;②生存时间长者KDR表达阳性率较低(r=-0.444,P<0.01);③VEGF与肿瘤MVD呈正相关(P<0.001),但是VEGF MVI与患者预后并无显著相关性。结论KDR在肿瘤细胞中的阳性表达率与髓母细胞瘤的预后呈负相关。髓母细胞瘤中KDR的表达水平可以作为判断病人预后的指标之一。

Expression of VEGF and KDR in medulloblastoma and its clinical significance

Abstract:Objective To probe the possible relationship between the expression of VEGF(Vascular endothelial growth factor) and KDR(kinase domain region, one of the VEGF receptors) in medulloblastoma and tumor mcrovascular density (MVD) and patient the prognosis. Methods 45 cases of medulloblastoma of the posterior fossa and ten cases of normal cerebellar tissue were studied via a combination of immunnohistochemical staining in formalin fixed paraffin - embeded sections. Results ①Both VEGF and KDR were expressed weakly in normal cerebellar tissue, while VEGF and KDR were either moderately or strongly ex pressed in most of the medulloblastoma. Moreover, KDR was expressed not only in the endothelial cells of tumor vasculature, but also in tumor cells. ②Correlation analysis indicated that expression of KDR was correlated singnificantly with survival time (r = -0.444, P < 0.01) ,③Correlation analysis also indicated that expression of VEGF was correlated singnificantly with MVD (Mi-crovascular density) , but neither VEGF nor MVD had significant correlation with survival time. Conclusion The expression level of KDR had a negative correlation with the prognosis of medulloblastoma. VEGF might influence the growth of both tumor cells and newly formed blood vessels in medulloblastoma.
