多点皮瓣固定法预防乳腺癌术后皮下积液效果观察

目的观察多点皮瓣固定法预防乳腺癌改良根治术后皮下积液的效果。方法152例乳腺癌患者按手术方法分为观察组和对照组,观察组采用多点皮瓣固定法,对照组采用传统皮瓣加压法。观察2组患者术后引流量变化、拔管时间、皮下积液的发生情况。结果观察组和对照组术后第1天的24 h引流量无差别(P>0.05),第3天和第5天的24 h引流量观察组明显低于对照组(P<0.01),观察组平均拔管时间4.46±0.56 d,皮下积液发生率为7.1%;对照组平均拔管时间8.74±0.67 d,皮下积液发生率为34.1%,2组间比较差异显著(P<0.05,P<0.01)。结论多点皮瓣固定法在乳腺癌改良根治术中应用,能显著减少皮下积液的发生率,有利于皮瓣的愈合。

Efficacy of multidrop skin flap fixation in preventing subcutaneouly fluidify of post breast cancer operation

Objective To investigate the effect of multidrop skin flap fixation in preventing subcutaneouly fluidify of post modified radical mastectomy.Methods 152 cases with breast cancer were divided into observed group and contronl group.70 cases in observed group were performed with multidrop skin flap fixation.82 cases in control group were performed with the traditional skin flap pressurizing.The amount of the effusion,the time of extubation and the incidence rate of subcutaneouly fluidify were observed.Results The first day the amount of subcutaneouly fluidify in 24 h of post operation was no difference between two groups.The third day and the fifth day the amount of subcutaneouly fluidify in 24 h were signficantly lower than control group(P<0.01).The observed group's mean time of extubation was 4.46±0.56d,the incidence rate of subcutaneouly fluidify was 7.1%.The control group's mean time of extubation was 8.74±0.67d,the incidence rate of subcutaneouly fluidify was 34.1%.There was significantly diference between two groups(P<0.05,P<0.01).Conclusion The multidrop skin flap fixation used in breast modified radical mastectomy can reduce the incidence rate of subcutaneouly fluidify significantly and make for the agglutination of skin flap.
