摘    要:目的探讨解剖M型超声心动网(AMM)诊断节段性室壁运动异常的临床价值。方法402例疑为冠心病拟行冠状动脉造影的患者,术前行AMM超声心动图检查,并分析AMM与冠状动脉造影诊断的一致性,比较不同程度狭窄情况下两种检查方法诊断的一致性,以及不同部位狭窄情况下两者诊断的一致性。结果当冠状动脉轻度狭窄时,AMM与冠状动脉造影诊断无高度一致;当冠状动脉重度狭窄时,AMM与冠状动脉造影诊断高度一致;冠脉不同部位狭窄情况下,AMM与冠状动脉造影诊断高度一致。结论AMM在重度冠脉狭窄与冠心病定位诊断方面与冠状动脉造影高度一致,具有较高的准确性,但对无明显或轻度狭窄冠心病的诊断与冠状动脉造影差距较大。

Abstract:Objective To explore the value of anatomic M-mode (AMM) echoeardiography in diagnosis of regional wall motion abnormalities (RWMA). Methods Four hundred and two patients with suspected coronary artery disease (CHD) underwent AMM before coronary angiography (CAG). The consistency of AMM and CAG was analyzed, the consistency of different degree and location of stenosis was compared. Results AMM had high sensitivity in detecting RWMA in patients with severe coronary artery stenosis, but had low sensitivity in patients with moderate stenosis or without stenosis. In another aspect, localization of the culprit coronary by AMM was highly consistent with CAG. Conclusion AMM is a convenient, noninvasive method to diagnose patients with severe coronary artery stenosis and determine the culprit coronary artery, highly consistent with CAG, but not for patients with moderate stenosis or without stenosis.
