比索洛尔治疗不稳定型心绞痛49例疗效分析

目的观察比索洛尔治疗不稳定型心绞痛(UA)的临床疗效及安全性。方法将96例不稳定型心绞痛患者分为治疗组49例和对照组47例,两组均给予常规治疗(拜阿司匹林100 mg口服,每天1次,低分子质量肝素钠5 000 U脐周皮下注射,每天1次,单硝酸异山梨酯缓释片60 mg口服,每天1次,阿托伐他汀20 mg每晚口服)。治疗组在常规治疗的基础上加用比索洛尔,起始量1.25 mg/d,每周递增1.25 mg,至5～10 mg/d,疗程1年。观察两组心绞痛症状及心电图改善情况、不良反应、心脏急性事件(心绞痛、心律失常、急性心肌梗死)发生率。结果两组患者治疗6、9个月,1年后的心绞痛症状和心电图表现均明显改善,但治疗组优于对照组,差异均有统计学意义(P<0.05);治疗组不良反应轻;治疗组降低心绞痛、心律失常发生率的效果亦优于对照组(P<0.05)。结论在常规治疗的基础上加用比索洛尔,只要掌握好适应证,规范用药,找到合适的剂量并坚持服药,比索洛尔能显示出良好的疗效和安全性。

Efficacy analysis on bisoprolol for treating unstable angina in 49 cases

Objective To observe the clinical efficacy and safety of bisoprolol for treating unstable angina（UA）.Methods 96 cases of UA were divided into the treatment group（49 cases） and the control group（47 cases）.Two groups received the routine treatment（oral Bayaspirin 100 mg,once daily,low molecular heparin sodium 5 000U by periumbilical subcutaneous injection,once daily,oral isosorbide mononitrate sustained release tablets 60 mg,once daily,oral atorvastatin 20 mg,once every night）.Based on the routine treatment,the treatment group was added with bisoprolol,initial dose of 1.25 mg/d,increasing by 1.25 mg per week,to 5～10 mg/d,with the treatment course of 1 year.To observe the angina symptoms,improvement of electrocardiogram,adverse reactions,incidence rate of acute cardiac events such as angina,arrhythmia,acute myocardial infarction in the two groups.Results After 6-month,9-month and 1-year treatment,the angina symptoms and electrocardiograph manifestations in the two groups were significantly improved,but the treatment group was superior to the control group,difference showing statistical significance（P〈0.05）.The adverse reactions in the treatment group were mild.The effect of decreasing the incidence rate of angina and arrhythmia in the treatment group was also better than the control group（P〈0.05）.Conculsion On the basis of routine treatment,adding bisoprolol can demonstrate the better curative effects and safety,if only mastering indications,standardizing medication,suitable dose and insisting medication.
