微创腔镜技术的腹部外科临床实践

目的 研究腹腔镜下外、妇科联合手术完成腹腔内多种病变切除的临床效果.方法 该院2009年1月至2012年2月共完成联合手术29例,胆囊、子宫切除术1例,腹腔镜胆囊切除术、腹腔镜胆总管探查术6例,胆囊、阑尾切除术8例,卵巢囊肿剥除或附件、胆囊切除术1例,脾囊肿切除术1例,肝硬化原发性腹膜炎探查引流术1例,腹部损伤探查术2例,输卵管异位妊娠切除术1例,阑尾切除术8例.结果 29例患者均顺利治愈、患者术后下床活动时间6～12h,平均8h;肠功能恢复(术后排气)时间10-62 h,平均26h;术后住院时间3～7d,平均5d.结论 腹腔镜手术可以切除多种病变,具有微创效果.

Clinical practice of minimally invasive endoscopic technique in abdominal surgery

Obective To study the clinical effects of laparoscopic surgical and gynecological joint operation for multiple intraperitoneal lesions resections. Methods 29 cases of joint operation were performed in our hospital from January 2009 to February 2012 ,including 1 case of cholecystectomy＋hysterectomy,6 cases of cholecystectomy＋common bite duct exploration, 8 cases of cholecystectomy＋appendectomy, 1 case of ovarian cystectomy or adnexectomy＋cholecysteetomy, l case of spleen cyst resection, 1 cases of exploration drainage for cirrhosis primary peri- tonitis, 2 cases of abdominal injury exploration, 1 case of fallopian tube ectopic pregnancy resection and 8 cases of appendectomy. Results 29 cases were successfully cured. The time to get out of bed for activity was 6-12 h （average 8 h）. The recovery time of intestinal function （postoperative exhaust time） was 10-62 h （average 26 h）. The postoperative hospitalization duration was 3-7 d （average 5 d）. Conclusion Laparoscopic surgery can be applied to multiple lesion resection with minimally invasive effect.
