腹腔镜与开腹胃癌根治术的安全性分析

目的:探讨分析腹腔镜与幵腹胃癌根治术的安全性及手术疗效。方法:选取120例胃癌患者作为研究対象,随机分为观察组(n=60)与对照组(n=60)。观察组行腹腔镜D2根治术,対照组行幵腹胃癌根治术。比较两组手术时间、手术出血量、手术切口长度、住院时间、手术安全性评价指标(淋巴结清扫数量、近切缘长度、远切缘长度)、术后并发症发生情况(切ロ感染、肺部感染、吻合ロ瘘、粘连性肠梗阻)。结果:观察组术中出血量、手术切ロ长度、住院时间、术后并发症发生率优于对照组(P0.05)。两组淋巴结清扫数量、切缘长度及手术时间差异无统计学意义(P0.05)。结论:腹腔镜胃癌D2根治术治疗胃癌具有较好的安全性,与开腹手术相比,术后患者康复较快,手术创伤较小,值得推广应用。

Safety analysis of laparoscopic and open radical gastrectomy for gastric cancer

Objective:To analyze the safety and operative therapeutic effect of laparoscopic and open radical gastrectomy in the treatment of gastric cancer. Methods:One hundred and twenty patients with gastric cancer treated from Feb. 2015 to Feb. 2016 were randomly divided into observation group (n=60) and control group (n=60) according to the method of random number table. The pa-tients in observation group were treated with laparoscopic D2 radical resection,and the patients in the control group were treated with open radical gastrectomy. The time of operation,the amount of blood loss,the length of incision,hospital stay,the operative safety param-eters ( the number of dissected lymph nodes,the length of the proximal and distal margin) ,the postoperative complications ( incision in-fection,pulmonary infection,anastomotic fistula,adhesive intestinal obstruction) were compared between the two groups. Results:The operative blood loss,incision length,hospital stay and incidence of postoperative complications of the observation group were significant-ly less than those of the control group (P<0. 05). There was no significant difference in the number of dissected lymph node,the length of the margin and the operation time between the two groups (P>0. 05). Conclusions:Laparoscopic D2 radical gastrectomy for gastric cancer has a better safety. Compared with open radical gastrectomy,laparoscopic procedure has advantages of faster recovery and fewer operative trauma,is worthy of popularization and application.
