经脐单孔与多孔腹腔镜胰体尾切除术的回顾性对比分析

目的:总结经脐单孔腹腔镜胰体尾切除术的临床经验,并与传统多孔腹腔镜胰体尾切除术进行回顾性对比分析,探讨其优缺点。方法:2009年12月至2016年2月为26例患者行经脐单孔腹腔镜胰体尾切除术,7例行传统多孔手术。对比分析两组患者围手术期数据及术后并发症。结果:单孔组与多孔组各有1例中转开腹;单孔组与多孔组手术时间[(164.3±58.8)min vs.(202.1±122.5)min]、术中失血量[(165.0±120.1)ml vs.(168.6±157.4)ml]、术后住院时间[(8.7±2.8)d vs.(9.0±3.0)d]、术后并发症发生率及保脾率差异无统计学意义(P0.05)。结论:对于有经验的腹腔镜外科医生,经脐单孔腹腔镜胰体尾切除术是安全、可行的,且单孔腹腔镜手术具有潜在的美容优势。

Retrospective comparative analysis of transumbilical single-incision laparoscopic distal pancreatectomy and conventional multi-incision laparoscopic surgery

Objective:To summarize the experience of transumbilical single-incision laparoscopic distal pancreatectomy (TUSI-LDP),and compare its related parameters with conventional muhi-incision laparoscopic distal pancreatectomy (C-LDP),and confirm the potential advantages of TUSI-LDP.Methods:From Dec.2009 to Feb.2016,26 patients underwent TUSI-LDP and 7 patients underwent C-LDP.The demographic data,operative parameters,and postoperative complications in the two groups were summarized and compared.Results:In the TUSI-LDP group and the C-LDP group,one patient was converted to open surgery in each group.The mean operating time and intraoperative blood loss in TUSI-LDP group was a little decreased,but there were no significant difference in these aspects [(164.3±58.8) min vs.(202.1 ±122.5) min,(165.0±120.1) mlvs.(168.6±157.4) ml,P＞0.05)].The mean postoperative hospital stay in TUSI-LDP group was a little shorter,and there was also no significant difference in this aspect [(8.7 ±2.8) d vs.(9.0 ±3.0) d,P ＞ 0.05].For the post-operation complications and the spleen-preserving rate,there were no significant differences in this two parameters.Conclusions:For experienced laparoscopic surgeons,TUSI-LDP is a safe and feasible procedure with similar perioperative outcomes to C-LDP.And excellent cosmetic effect is perhaps its potential advantage.
