摘    要:目的:探讨悬吊式无气腹腹腔镜子宫肌瘤切除术对患者免疫功能与卵巢功能的影响。方法:选择2014年1月至2014年12月就诊的76例子宫肌瘤切除术患者作为研究对象,采用随机数字表法分为观察组与对照组,每组38例。对照组行气腹腹腔镜手术,观察组行经腹壁皮下悬吊式腹腔镜子宫肌瘤切除术。对比分析两组患者手术情况、免疫功能、卵巢功能。结果:观察组手术时间、术中出血量明显低于对照组,差异有统计学意义(t=2.462~3.337,P0.01);术后第3天,观察组CD3~+、CD4~+及CD4/CD8比值显著高于对照组(t=2.316~4.191,P0.05);术后6个月,观察组雌二醇、孕酮水平低于对照组,促卵泡激素、黄体生成素明显高于对照组,组间相比差异有统计学意义(t=2.153~2.458,P0.05)。结论:悬吊式无气腹腹腔镜子宫肌瘤切除术有助于减轻手术创伤,保护机体免疫功能,促进术后卵巢功能的恢复。

The impact of suspended gasless laparoscopic myomectomy on immune function and ovarian function
Abstract:Objective:To study the effect of suspended gasless laparoscopic myomectomy on immune function and ovarian func-tion. Methods:From Jan. 2014 to Dec. 2014,76 patients who underwent myomectomy were divided into observation group and control group with random number table,the control group underwent pneumoperitoneum laparoscopic surgery,observation group received sus-pended gasless laparoscopic myomectomy,then operative data,immune function and ovarian function were compared between the two groups. Results:The operation time,blood loss of observation group were significantly lower than those of the control group (t=2. 462-3. 337,P<0. 01);On the third day after surgery,CD3+,CD4+ and CD4/CD8 ratios in observation group were significantly higher than those in the control group (t=2. 316-4. 191,P<0. 05). In the postoperative 6 months,level of estradiol and progesterone in the observa-tion group were significantly lower than those in the control group,follicle-stimulating hormone and luteotropic hormone were significant-ly higher than those of the control group (t=2. 153-2. 458,P<0. 05). Conclusions:Suspended gasless laparoscopic myomectomy helps to reduce the surgical trauma,protect the immune function,and promote postoperative recovery of ovarian function.
