原发性胃癌与胃淋巴瘤的影像学特点与鉴别诊断

目的总结原发胃癌及胃淋巴瘤的影像学表现特点及二者的鉴别诊断要点，减少二者的漏误诊率。方法对53例手术及病理均证实胃恶性肿瘤的患者分析其胃肠钡餐、CT的影像学表现。结果53例病例中有30例最终病检证实为原发胃癌，23例证实为胃淋巴瘤。临床表现多为上腹包块、腹痛、消瘦、恶心、呕吐，影像学表现主要是肿块、充盈缺损、龛影等。结论原发胃癌与胃淋巴瘤具有许多相同或相似的影像学表现，但是二者在胃肠钡餐造影、胃部CT等检查的影像学表现中又有各自的特点，掌握这些特点可以使原发胃癌与胃淋巴瘤的误诊率大大降低。

Imaging in the differential diagnosis of primary gastric cancer and gastric lymphoma

Objective Through the primary gastric cancer and gastric lymphoma image analysis, to summarize the primary gastric cancer and gastric lymphoma imaging features, further analyze the differential diagnosis between the two and reduce both missed and the rate of misdiagnosis. Methods 53 cases with gastric cancer were analyzed including the gastrointestinal barium, CT imaging and fiber endoscopy. Results Of 53 cases, there were 30 cases in the final pathologic examination confirmed primary gastric cancer and 23 cases of confirmed lymphoma. The clinical symptoms and imaging performance of the patients were different, but with similar properties, such as clinical manifestations of abdominal mass, abdominal pain, weight loss, nausea , vomiting , imaging performance is mainly mass, filling defects, Kan Ying and so on. Conclusion The primary gastric cancer and gastric lymphoma has many of the same or similar imaging findings, but both imaging findings in barium stomach , stomach fiber endoscopy, CT examination of the stomach have their own characteristics, to master these features can make primary gastric lymphoma and gastric missed greatly reduced rate and the rate of misdiagnosis.
