精阜近端电切标识在前列腺汽化电切中的临床研究

目的探讨精阜近端电切标识在经尿道前列腺汽化电切术(TUVP)治疗良性前列腺增生(BPH)中的临床应用。方法将60例BPH患者按随机数字表法分为观察组与对照组,每组30例。观察组采用精阜近端电切标识法行TUVP,对照组使用Nesbit法行TUVP。对2组术中的各项参数(手术时间、切除前列腺重量、血Na+下降程度等)、疗效冲洗、留置导尿时间,膀胱痉挛、前列腺国际症状评分(IPSS)、残余尿量(PVR)、最大尿流率(Qmax)、生活质量评分(QOL)]及并发症经尿道电切综合征(TURS)、急性附睾炎、尿道外口狭窄、勃起功能障碍等]进行比较。结果观察组手术时间、Na+下降值、IPSS、PVR、Qmax、QOL及并发症的发生率均低于对照组,差异均有统计学意义(均P<0.05)。结论精阜近端电切标识法行TUVP具有可行性、安全性、有效性,手术时间短、术后恢复快、并发症少及创伤小的优势,尤适合于心肺功能减退的患者,值得借鉴。

Application of Proximal Verumontanum Margin Marked Method in Transurethral Electrovaporization of Prostate

Objective To study the clinical application of proximal verumontanum margin marked method in the transurethral electrovaporization of prostate（TUVP） for benign prostatic hyperplasia（BPH）.Methods Sixty patients with BPH were randomly assigned to receive TUVP with either proximal verumontanum margin marked method（observation group,n=30） or Nesbit＇s method（control group,n=30）.The operation time,resected prostate tissue weight,decreased value of sodium ion,bladder irrigation time,catheterization time,cystospasm,prostate international symptom score（IPSS）,post-voiding residual urine volume（PVR）,maximal urinary flow rate（Qmax）,quality of life（QOL）,transurethral resection syndrome（TURS）,acute epididymitis,meatal stenosis and erectile dysfunction were compared between the two groups.Results Compared with control group,operation time,decreased value of sodium ion,IPSS,PVR,Qmax,QOL and incidence of complications significantly decreased in observation group（all P0.05）.Conclusion The TUVP with proximal verumontanum margin marked method is feasible,safe and effective and leads to shorter operation time,rapid postoperative recovery,less complications and minimal trauma in patients with BPH,especially in patients with cardiopulmonary dysfunction.
