急性早幼粒细胞白血病（M_3）亚型的细胞形态及临床特征观察

对２２例急性早幼粒细胞白血病（ＡＮＬＬ－Ｍ＿３）患者进行了细胞形态、超微结构、形态测量、细胞遗传学研究及临床特征分析，其中粗颗粒型（Ｍ＿３ａ）１０例、细颗粒型（Ｍ＿３ｂ）１２例。结果显示，Ｍ＿３ｂ型患者骨髓及外周血有核细胞中早幼粒细胞的比例较高，骨髓增生度高，白血病细胞核畸形明显。其胞浆颗粒不仅体积小、数量少，且大部分颗粒充盈不全，染色体异常检出率亦高于Ｍ＿３ａ亚型。除Ｍ＿３ｂ亚型患者初诊外周血白细胞计数较高外，两组患者临床表现相近。

MORPHOLOGICAL AND CLINICAL CHARACTERISTICS OFSUBTYPES OF ACUTE PROMYELOCYTIC LEUKEMIN(ANLL-M_3)

22 cases of acute promyelocytic leukemia were studied in clinical and morphologicalcharacteristics,ultrastructure,morphometry,and cytogenetics.Among them,10 werehypergranule APL(M_3a) and 12 were microgranule APL (M_3b).Comparing with the others,M_3b patients had higher percentage of promyelocyte in NEC (all neucleated cell)of bone marrowand peripheral blood,its bone marrow had higher cellularity.N ucleus irregularity in M_3bleukemic cells was more evident, its cytoplasma contains a few of fine dustlike granules withmany remained at unfull stage ultrastructurally. The chromosomal abnormality rate of M_3bleukemic cell was also higher than that of M_3a,The clinical features were nearly the same,exceptM_3b patients had higher leucocyte count in peripheral blood at their first diagnosis.
