应用功能核磁共振成像技术分析针刺加康复运动疗法促进脑瘫大脑再塑的作用机制

摘    要:目的:应用fMRI技术动态观察研究针刺加康复运动疗法对脑瘫大鼠模型大脑再塑、功能重组的影响,以阐明脑瘫大鼠大脑组织缺血缺氧后的组织结构、细胞功能与脑瘫肢体运动功能恢复的关系,探讨针刺加康复运动疗法促进脑瘫肢体运动功能恢复的作用机制。方法:通过构建脑瘫大鼠缺血缺氧模型,应用fMRI技术动态追踪检测脑组织病变区细胞功能代谢的动态变化情况;并结合透射电镜对脑组织的形态结构、超微病理检测和运动功能评估结果分析脑瘫鼠经针刺加康复运动治疗后肢体运动功能恢复与大脑病变区超微组织形态结构和细胞功能代谢的动态变化关系。结果:针刺加康复运动疗法可以有效地恢复脑瘫肢体运动功能,并在改善大脑病变区细胞功能代谢的同时,对脑组织病变区的超微病理结构亦具有明显的促进其恢复作用。fMRI技术动态追踪检测、透射电镜超微病理检测结果与运动功能评估结果呈正相关。针刺加康复运动疗法有效促进脑瘫肢体运动功能恢复的机制,是通过恢复大脑病变皮质区脑细胞的线粒体、突触数及密度,激活脑细胞功能代谢来实现的,主要基于大脑的再塑与功能重组。结论:fMRI技术对脑瘫康复过程中疗效结果的无创追踪分析与评估亦具有重要的实际意义。

Mechanism on Cerebral Plasticity of Cerebral Palsy with the Treatment of Acupuncture and Rehabilitation by Methods of Functional Magnetic Resonance Imaging

Abstract:Objective:To investigate the effect of acupuncture and rehabilitation on cerebral plasticity and func- tional reorganization of cerebral palsy rats, clarify the relation between tissue' s morphology, cell function and the recovery of movememt function of cerebral palsy after cerebral ischemia - hypoxia, and reveal the mecha- nism of acupuncture and rehabilitation therapy in the side of improving the function of body movememt of cere- bral palsy. Methods : The rat models of cerebral ischemia - hypoxia were constructed, and the fMRI techniqueswere used to test the dynamic change of cell metabolism of brain lesion area, combined with the TEM tech- niques to detect the brain tissue' s morphological structure and ultrastructure , the movement function assess- ment results which was used to analyze the dynamic change about body function with ultramorphologies, cell metabolism after the treatment by acupuncture and rehabilitation on cerebral palsy rats. Results: Acupuncture and rehabilitation which recover the function of body movememt of cerebral palsy, could improve the cell me- tabolism, besides promoting the recovery of brain tissue' s ultramicro pathological changes observably. The fM- RI techniques dynamic tracking detection and the test result of ultramicro pathological by transmission electron microscop had a positive relationship with the score of motor function. The mechanism on cerebral plasticity which recovered the function of body movememt of cerebral palsy with acupuncture and rehabilitation were based on cerebral plasticity and functional reorganization. Conclusion:The fMRI techniques have a great signif- icance in assessment and trace analysis for the efficacy on the rehabilitation of cerebral palsy.
