142例鞍区占位病变手术治疗总结与体会

目的探讨鞍区占位病变手术治疗方案。方法回顾性分析我科收治的142例鞍区占位病变手术治疗的临床资料。结果术后并发尿崩74例,电解质紊乱45例,高热11例,垂体低功11例,视力恶化4例,上消化道出血5例,脑脊液鼻漏5例,癫痫3例,高血糖2例,颅内血肿2例,死亡7例。大部分病例随访8~33月。结论选择合适的手术入路,熟悉鞍区显微解剖,合理应用内镜及导航等新技术,能有效减少并发症的发生,提高手术疗效。

Summary of surgical treatment to 142 patients with mass lesion in sellar region

Objective To explore the surgical treatment for mass lesions in sellar region. Methods The clinical data of 142 patients with mass lesion in sellar region were analyzed retrospectively. Results Of 142 patients,74 had diabetes insipidus,45 electrolyte disorder, 11 high fever, 11 hypopituitarism, 4 deterioration of vision and visual field, 5 upper digestive tract, 5 CSF fitula, ,3 epilepsy, 2 hyperglycemia, 2 intracranial hematoma and 7 died. A large part of patients recieved the follow-up from 8 to 33 months. Conlusions The suitable operative approach, familiar with microanatomy in sellar region, application of neuroendoscopy and navigation techniques, are highly important to decrease the complications and improve the curative effects.
