老年慢性阻塞性肺疾病患者C反应蛋白水平变化及其与胰岛素抵抗的相关性

目的 探讨老年慢性阻塞性肺疾病（COPD）患者体内C反应蛋白（CRP）水平变化及其与胰岛素抵抗（IR）的相关性.方法 选取本院2011年8月至2013年8月收治住院的老年COPD患者93例,根据病情进展情况分为急性进展期48例（AECOPD组）和稳定期45例（SCOPD组）;另选取同期健康体检者45例作为对照组.测定CRP、空腹血糖（FPG）、空腹胰岛素（FIN）、餐后2h血糖（2h PBG）,计算IR指数（HOMA-IR）.结果 各实验室检测指标及HOMA-IR依次由健康组、SCOPD组、AECOPD组逐渐增高,3组比较差异均有统计学意义（P＜0.05）.Pearson相关性分析结果表明,AECOPD组患者CRP与FPG、FIN、2h PBG及HOMA-IR呈现正相关性（r=0.443、0.532、0.512及0.601,均P＜0.05）.结论 IR可见于COPD患者体内,并以AECOPD患者体内最为明显,IR的发生与CRP的表达密切相关。

The C-reactive protein levels of elderly patients with chronic obstructive pulmonary disease and its correlation between insulin resistance

Objective To study the C-reactive protein（CRP）levels of elderly patients with chronic obstructive pulmonary disease（COPD）and its correlation between insulin resistance（IR）.Methods A total of 93 cases of elderly patients with COPD in department of internal medicine in our hospital from August 2011 to August 2013 were divided into acute progressive period of 48cases（AECOPD group）and 45 cases of stable period（SCOPD group）according to the progress of the disease.And another 45 cases of healthy people were selected as control group.The CRP,fasting blood glucose（FPG）,fasting insulin（FIN）,2hpostprandial blood glucose（2hPBG）and IR index（HOMA-IR）were determined.Results All the detection index and HOMA-IR of each lab consists of health group,SCOPD group and AECOPD group were gradually increased（P〈0.05）.The Pearson correlation analysis showed that,there was positive correlation of CRP between the FPG,FIN,2hPBG and HOMA-IR in patients with AECOPD（r=0.443,0.532,0.512 and 0.601,all P〈0.05）.Conclusion IR can be found in COPD patients,and AECOPD patients is the most obvious.The occurance of IR is closely related to the epression of CRP.
