钬激光联合肾筋膜扩张器治疗男性尿道狭窄体会

目的：探讨联合应用钬激光、肾筋膜扩张器治疗尿道狭窄的临床操作方法及应用价值。方法：应用肾筋膜扩张器对33例尿道狭窄患者行尿道狭窄扩张术,27例尿道未闭锁者直接留置斑马导丝,6例尿道闭锁者应用输尿管镜配合钬激光打通尿道后留置斑马导丝,应用F8~F22肾筋膜扩张器依次扩张狭窄尿道。15例有较多瘢痕组织或狭窄段较长者应用钬激光修整。结果：33例病例1个月后拔除尿管排尿症状较前明显改善,术后定时尿道扩张后排尿舒畅,无并发症。结论：钬激光联合肾筋膜扩张器治疗男性尿道狭窄安全性高,创伤小,疗效好,值得推广采用。

The Clinical Application of Holmium Laser Combination with Renal Sheath Dilators in Treatment of Urethral Stenosis

Objective：To study the clinical methods and application value of Holmium laser in combination with renal sheath dilators in treatment of the urethrostenosis.Method：33 cases with urethrostenosis were underwent dilatation of urethra with the renal sheath dilators.The zebra guided wire was placed directly in 27 cases with no urethratresia.Ureteroscopy with the holmium laser was used to open up urethra and zebra quided wire was placed directty.In 6 cases with urethratresia,then the urethrostenosis were dilated with the dilators from 8F to 22F gradually.Among them,15 cases with longer stricture or more scar underwent trimming with Holmium laser.Result：The procedures of combination of ureteroscope and renal sheath dilator were compleleted satisfactorily.Conclusion：Holmium laser in combination with renal sheath dilators in the treatment the urethral stenosis safe,simple and effective therapeutic new method and it should be popularized.
