摘    要:目的 评价DWI动态监测新型血管阻断剂M410治疗兔VX2种植性肝癌的效果。方法 建立28只兔VX2种植性肝癌模型,随机等分为2组,分别静脉注射M410(实验组)和生理盐水(对照组)。两组于给药前、给药后4 h、1天、4天、7天、14天分别行MR平扫及DWI检查,并于各时间点取肿瘤组织行HE染色和CD34免疫组化检查,观察肿瘤体积、肿瘤整体及实质平均ADC值及病理变化。结果 实验组肿瘤体积较对照组增长缓慢,给药后4~14天各时间点两组肿瘤体积差异均有统计学意义(P均<0.05)。肿瘤整体及实质平均ADC值:给药后1天实验组低于对照组,4天实验组高于对照组,差异均有统计学意义(P<0.05)。ADC值改变与肿瘤组织病理学结果相符。结论 DWI可动态反映新型血管阻断剂M410治疗兔VX2种植性肝癌后肿瘤分子水平的变化,通过测量肿瘤ADC值可早期监测M410的疗效。

Abstract:Objective To quantitatively monitor the therapeutic effect of the vascular disrupting agents (M410) on rabbit VX2 liver tumor with DWI. Methods Twenty-eight rabbits implanted VX2 of liver tumor model were successfully established and randomly divided into two groups (treatment group and control group). Conventional MRI and DWI were performed in a 3.0T MR unit at pretreatment, 4 h, 1 day, 4 days, 7 days and 14 days post-treatment. HE and CD34 immunohistochemical examination were performed at each time point. The changes of tumor volume, average ADC values of the whole tumor and parenchyma of the tumor, as well as pathological results at each time point between the two groups were evaluated. Results The tumor volume had statistically significant on 4-14 days in both groups post-treatmert (P<0.05). Average ADC values of the whole tumor and parenchyma of the tumor in treatment group was lower than that of control group at 1 day post-treatment, but higher at 4 days post-treatment (all P<0.05). Changes of ADC value was consistent with pathological results of tumor tissue. Conclusion The molecular changes in treatment of rabbit liver VX2 tumor using M410 can be dynamically reflected by DW MRI, and early monitor the efficacy of the M410 through measuring the ADC values of the tumor.
