维持性血液透析患者心血管疾病危险因素分析

目的　分析维持性血液透析患者心血管疾病发生的危险因素。方法　研究对象为 4 2例维持性血液透析患者 ,2 0例肾功能正常的心血管疾病患者。血液透析患者分心血管疾病组 15例和非心血管疾病组 2 7例。测量每位研究对象血压 ,计算体重指数 (BMI) ,检测血肌酐 (SCr)、白蛋白 (Alb)、前白蛋白 (PA)、胆固醇 (TCH)、血红蛋白 (Hb)和铁蛋白 (Ferritin)。结果　肾功能正常的心血管疾病组BMI、Alb、PA、TCH和Hb明显高于血液透析患者 (P <0 .0 1) ,Ferritin水平明显低于血液透析患者 (P <0 .0 1)。血液透析心血管疾病组和非心血管疾病组比较 ,前者BMI、SCr(透析前 )、Alb、PA、TCH和Hb明显低于后者 (P <0 .0 1) ,前者Fer ritin水平、收缩压 >16 0mmHg(1mmHg=0 .133kpa)和 <110mmHg的比例明显高于后者 (P <0 .0 1)。结论　维持性血液透析患者心血管疾病发生的危险因素可能包括低体重指数、透析前血肌酐水平降低、低 (前 )白蛋白血症、低胆固醇血症、低血红蛋白、高铁蛋白血症、收缩压过高或过低。

Analysis of cardiovascular risk factors in maintenance hemodialysis patients

Objective To analyze cardiovascular risk factors in maintenance hemodialysis(MHD)patients. Methods Fourty-two MHD cases and 20 cardiovascular disease (CVD) cases without renal failure were enrolled. The 42 MHD patients were divided int o CVD group and non- CVD group. Blood pressure, body mass index (BMI), serum cre atinine(SCr), albumin(Alb), pre-albumin(PA), cholesterol(TCH), hemoglobin(Hb), a nd ferritin were detected. Results The data of BMI, Alb, PA, TCH, and Hb of CVD without ren al failure was higher than that of MHD (P<0.01). The level of fe rritin was lower than that of MHD (P<0.01). In MHD, the level of BMI, Scr of pre-HD, Alb, PA, TCH, and Hb of CVD group was lower than that of no n-CVD group (P<0.01). The level of serum ferritin and the ratio of SBP higher than 160mmHg and lower than 110mmHg of CVD group were higher than those of non-CVD group (P < 0.01). Conclusion The cardiovascular risk factors in MHD patients prob ably included low body mass index, low serum creatinine, hypoalbuminemia, low pr e-albumin, hypocholesterolemia, low hemoglobin, high ferritin, and hypertension or hypotension.
