肾上腺节细胞神经瘤的螺旋CT表现及诊断价值

目的:探讨肾上腺节细胞神经瘤的螺旋CT表现及诊断价值。方法:回顾性分析7例经手术病理证实的肾上腺节细胞神经瘤患者的螺旋CT表现及临床病理资料。结果:7例肿块呈圆形或椭圆形,边界清晰,密度均匀,5例肿块内见多发或单发细小斑点状钙化。6例肿块最大径在5cm以上者,肿瘤受邻近器官及血管不同程度挤压而呈铸形生长。7例增强扫描肾皮质期肿块无明显强化,肾实质期轻度强化,强化程度10～15HU。7例肿块无论平扫及增强扫描均未见明显坏死囊变征象。结论:铸形生长、细小钙化、轻度延迟强化及肿块无明显坏死囊变为肾上腺节细胞神经瘤的螺旋CT表现特征,可在术前作出诊断及鉴别诊断。

Spiral CT Appearances of Adrenal Ganglioneuroma and Diagnostic Value of CT

Objective:To investigate the spiral CT appearances of adrenal ganglioneuroma and diagnostic value of CT.Methods:Spiral CT appearances and clinical pathological data of 7 patients with ganglioneuroma diagnosed pathologically were retrospectively reviewed.Results:Tumors appeared as round or oval,well defined and homogeneous mass in 7 cases.There were multiple or single punctate calcification in 5 cases.Because of the extrusion with different degrees by the vicinal organ or blood vessel,6 tumors with the maximal diameter more than 5cm showed a cast mould growth.Enhanced scanning showed no evident enhancement in the cortico-medullary phase in 7 cases,but slight enhancement in the parenchymal phase (10~15 HU).There was no obvious sign of either necrosis or cystic change in both plain scanning and enhanced scanning.Conclusion:Spiral CT appearances of adrenal ganglioneuroma is cast-like growth,punctate calcification,slight delayed enhancement and no evident necrosis or cystic change.It is possible to make diagnosis and differential diagnosis before operation.
