骨髓巨核细胞数量在激素无效免疫性血小板减少性紫癜患者中的价值

摘    要:目的 探讨骨髓巨核细胞数量与特发性血小板减少性紫癜(ITP)激素耐药的关系,为ITP治疗提供一定理论依据。方法将126例新发患者分成巨核细胞增多组(A组,77例)和巨核细胞正常或减少组(B组,49例),观察两组激素治疗有效率及激素无效患者重组人白细胞介素-11(rhIL-11)治疗有效率。结果A组激素治疗有效65例,无效12例,有效率84、4%;B组激素治疗有效34例,无效15例,有效率69.4%;A组有效率高于B组,差异具有显著性。A组激素治疗无效12例中,rhlL-11治疗有效4例,有效率33.3%;B组激素治疗无效15例中,rhIL-11治疗有效11例,有效率73.3%;B组有效率高于A组,差异具有显著性。结论骨髓巨核细胞减少的患者较巨核细胞增多患者更趋于对激素耐药,但对rhIL-11治疗却较敏感。

Abstract:Objective To clarify the curative significance of bone marrow megakaryocyte counts in patients with idiopathic thrombocytopenic purpura in order to serve as the basis for its U'eatment. Methods 126 patients were classified into two groups according to their megakary, ocytic counts on hone marrow smears (77 patients in group A with megakary, ocyfic counts over 35, and 49 patients in group B with megakaryocytic counts less than 35). Results There was significant difference in response to glucocorticoids between group A and group B(84.4% vs. 69.4%, P 〈 0. 05 ), and the efficaey in patients of group A was significantly higher than that of patients in group B. There was significant difference in response to interleukin - 11 between those failed to respond with glucoeorticoid in group B and group A(73.3% vs. 33.3%, P 〈 0.05). Conclusion Patients with megakaryocytic counts less than 35 tend to be less effective in response to glucocorticoid, but they are more sensitive in response to interleukin - 11.
