动态监测PCT对化脓性脑膜炎婴幼儿治疗转归的影响

目的:动态监测血清中降钙素原(PCT)的含量,探讨其在化脓性脑膜炎(化脑)婴幼儿治疗转归中的应用价值。方法:收集郑州人民医院儿科60例化脑婴幼儿临床资料,根据患者临床预后分为痊愈组、后遗症组和死亡组,比较3组患儿PCT变化。结果:60例患儿中,痊愈30例,留有不同程度的后遗症患儿19例,死亡11例。3组患儿的PCT基础值(确诊时)比较差异无统计学意义(P>0.05)。但3组患儿治疗后PCT比较差异有统计学意义(P<0.01)。痊愈组PCT治疗后较治疗前均降低,后遗症组有8例降低,余均升高,但降低幅度却小于痊愈组(P<0.01),死亡组均升高。结论:PCT水平的动态监测对判断化脓性脑膜炎婴幼儿的预后具有一定的应用价值。

The role of dynamic monitoring PCT in the prognostic evaluation of purulent meningitis in infants

Objective： To explore the value of dynamic monitoring serum procalcitonin（ PCT） content in evaluating prognosis of infant patients with purulent meningitis. Methods： The clinical data of 60 cases of infant meningitis in Zhengzhou People＇s Hospital were collected and divided into three groups according to prognosis,the groups including cured group,sequelae group,and death group,furthermore,the changes of PCT level in three groups were compared. Results： Among the60 patients,30 cases were cured,19 cases remained varying degrees of sequelae and 11 died. Although PCT values in three groups was not statistically different in the time of diagnosed（ P 0. 05）,the difference was statistically significant（ P 0. 01） after treatment. After treatment,PCT values were reduced in all compared with that of pretherapy in cured group,with PCT values in 8 of 19 cases were reduced compared to that of pretherapy in sequelae group. However,PCT values were elevated in 11 cases of sequelae group and all in death group compared to that of pretherapy. A decrease of PCT mean in sequelae group was smaller than cured group（ P 0. 01）.Conclusion： Dynamic monitoring PCT levels has important application values on the judgment of prognosis in infants with meningitis.
