二甲双胍和罗格列酮治疗多囊卵巢综合征的临床疗效观察

目的:比较二甲双胍和罗格列酮治疗胰岛素抵抗(Insulin resistance,IR)的多囊卵巢综合征(Polycystic ovarian syndrome,Pcos)患者内分泌、代谢异常的疗效.方法:将45例多囊卵巢综合征伴胰岛素抵抗的患者分为两组,A组给予罗格列酮4 m#dB组给予二甲双胍850mg/d,治疗4个月,比较治疗前后体重指数(Body mass index,BMI)、腰臀比(Waist-to-hipratio,WHR)、内分泌参数、HomaIR和血脂的变化.结果:两者治疗后,均能使血清睾酮(Testosterone,T)、黄体生成素(Luteinizing hormone,LH)、LH/FSH、空腹胰岛素(Fasting insulin,FINS)、2 h胰岛素(2 hINS)、2 h血糖(2 hPG)、低密度脂蛋白胆固醇(Low-density lipoprotein,LDL)的浓度明显下降,Homa IR明显改善(P<0.05).二甲双胍能降低BMI指数,而罗格列酮能更有效降低T、FINS、2 hINS、Homa IR.结论:罗格列酮比二甲双胍更能改善PCOS患者的胰岛素抵抗,适用于胰岛素抵抗较重的PCOS患者;二甲双胍有降低体重作用,适用于肥胖型PCOS伴胰岛素抵抗不严重者.

Effects of metformin and rosiglitazone in the treatment of polycystic ovarian syndrome with insulin resistance
