支架置入并化疗、放疗治疗恶性气管狭窄

目的:评价支架置入并化疗、放疗治疗恶性气管狭窄的临床效果。方法:对14例气管或支气管癌、食管癌、肺癌、纵隔肿瘤等恶性肿瘤致气管狭窄的患者.在X线下经纤维支气管镜放置镍钛记忆合金支架。所有的患者进行了后续的化学治疗和放射治疗。结果:14例患者均顺利置入支架,呼吸困难即刻好转。置入支架后,5例单纯行动脉化疗,4例单纯行放疗,5例先行动脉化疗1次～2次后再行放疗。7例支架置入后2个月～20个月死于肿瘤晚期全身衰竭及远处转移,平均生存8个月。余7例随访3个月～34个月仍存活,无呼吸困难及支架移位。结论:对于恶性气管狭窄的患者,支架置入是一种安全有效的方法,结合后续的化、放疗才能延长患者的生存时间。

Metallic stent implantation with chemotherapy and radiotherapy for malignant airway obstruction

Objective:To evaluate the clinical efficacy of metallic stent implantation with chemotherapy and radio-therapy in the treatment of malignant airway obstruction. Methods: Metallic stents were placed through bronchscope under x-ray in 14 cases with airway obstruction caused by malignant tumors (tracheal or bronchial carcinoma, esophageal cancer.lung cancer.and mediasternal tumor). All 14 cases underwent additional chemotherapy and radio-therapy. Results:ln 14 cases,metallic stents were successively introduced and shortness of breath was relieved imme-diately. After stent placement.inter-arterial chemotherapy was performed in 5 cases,radiotherapy in 4 cases and both inter-arterial chemotherapy and radiotherapy in 5 cases. 7 cases died from systemic failure and distance metastasis of advanced tumors with a mean survival period of 8 months(2 - 20 months). Another 7 cases remained alive at 3~26 months after stent placement.the airway was patent.and the stent remained in the original position. Conclusion:lt is very safe and effective method to treat malignant airway obstruction by means of metallic stent,additional chemother-apy and radiotherapy should be considered.
