肺炎衣原体增加糖尿病患者脑出血危险性的研究

摘    要:目的探讨肺炎衣原体(Cp)感染与脑出血的关系。方法 112例脑出血患者和82例非脑出血患者,应用酶联免疫吸附试验检测血液中的Cp特异性IgG抗体(CpIgG)。所有个体分为有、无脑出血,有、无脑出血危险因素(CpIgG抗体( )、年老、男性、吸烟、糖尿病、血脂紊乱、高血压、较少体力劳动和体重指数),比较有、无脑出血患者各种危险因素差别;并应用logistic回归分析,得出调整比值比(OR)值,用于评估Cp对脑出血的危险性。所有病例分为CpIgG阳性组与CpIgG阴性组,了解Cp 感染对脑出血危险因素的调整OR值变化影响。结果脑出血组CpIgG阳性率29.3%,对照组 CpIgG阳性率19.5%,差异显著(P<0.05)。logistic回归分析显示,CpIgG阳性与脑出血的调整OR 值为1.154(P>0.05);CpIgG阳性组糖尿病的调整OR值为4.720(P<0.05),CpIgG阴性组糖尿病调整OR值为1.202(P>0.05)。结论 Cp感染与脑出血有关,但不是脑出血发病的独立危险因素, 却加大糖尿病发生脑出血的危险。

Abstract:Objective To study the relationship between Chlamydia pneumoniae infection and hematencephalon. Methods In this cross-sectional study,112 patients with hematencephalon and 82 controls were tested serologically for specific CpIgG antibody with ELISA assay. The risk factors for hematencephalon were analysed included CpIgG ( ), aging, male, smoking, diabetes, hypertension, hyperlipidemia, mental labor and a high body mass index. Every risk factor between hematencephalon patients and controls was compared, respectively. The adjusted OR value was obtained by means of logistic regression analysis and then applied to the evaluation of hematencephalon risk from Cp. Results The prevalence of CpIgG seropositivity in patients with hematencephalon was 29.3%, which was significantly higher than 19.5% in the controls (P<0.05). logistic regression analysis demonstrated only a weak association between CpIgG seropositivity and hematencephalon after adjustment for confounding covariates, with the adjusted OR 1.154 (P>0.05). In those with CpInG seropositivity, adjusted OR of diabetes was much higher (4.720, P<0.05) than those with CpInG seronenativity (1.202, P>0.05). This indicated that CP infection could increase the risk of diabetes to develop into hematencephalon. Conclusion CP infection may play some worse role in the development of hematencephalon, and may increase the risk for hematencephalon in the patients with diabetes, though it is not an independent risk factor.
