颅骨缺损伴脑积水同期手术的临床研究

目的:探讨早期一次性颅骨修补及脑室腹腔分流在颅骨缺损伴脑积水治疗中的作用。方法:回顾性分析2005年12月至2007年12月同期行颅骨修补及脑室腹腔分流的脑外伤患者36例,对其临床资料、并发症及预后进行总结。结果:本组术后并发分流管阻塞1例,皮下积液1例,颅内感染1例;术后意识及神经功能障碍不同程度改善33例(91.67%);术后3~6个月进行GCS评分判定预后,恢复良好24例(66.67%);无手术死亡病例。结论:早期并同期行颅骨修补及脑室腹腔分流术并发症少,可明显缓解患者意识及神经功能障碍,提高患者预后,值得临床推广应用。

Clinical Research on Synchronization Operation of Patients with Skull Defect Complicated Hydrocephalus

Objective:To explore the role of early synchronization cranioplasty and ventriculoperitoneal shunt on the treatment of traumatic brain injury with hydrocephalus and skull defect.Methods:During December 2005 and December 2007,the clinical records,complication and prognosis of 36 patients with hydrocephalus complicated by skull defect with synchronization operation of cranioplasty and ventriculoperitoneal shunt were analyzed retrospectively.Results:The clinical complications of the patients were block of shunt tube in 1 case,subcutaneouly hydrops in 1 case,and intracranial infection in 1 case.The outcome of the patients was 33 cases(91.67%) improved obviously with consciousness and nerve function.After operation during 3 and 6 months,the 24 cases(66.67%)recovered well.No case was dead due to operation.Conclusions:There are fewer complications with operation of early hydrocephalus cranioplasty and ventriculoperitoneal shunt.And the operation can improve consciousness and nerve function and the prognosis.It is worth generalizing in clinic.
