产妇硬膜外麻醉穿刺负压现象临床表现观察

目的探讨妊娠影响硬膜外麻醉穿刺负压现象临床表现的机制及其意义。方法随机抽取于L2-3椎间隙行硬膜外麻醉女性患者100例，按照是否妊娠分成产妇组（LW）和非孕妇女组（NG）两组，每组50例。均选用18号硬膜外麻醉穿刺针穿刺，采用玻管法测试硬膜外间隙负压，测试结果分为有负压出现阳性（＋）、无负压出现阴性（-）和穿透硬脊膜测试失败（±）。结果两组比较产妇组负压阳性率明显偏低（P〉0．05），而且两组负压阳性患者继续进针刺穿硬脊膜的概率产妇组明显高于非孕妇女组（P〉0．05）。结论负压现象对于产妇硬膜外麻醉，不仅是判断穿刺针是否进入硬膜外间隙重要指征，而且应该更多地被看作麻醉操作可能会刺穿硬脊膜的一种警示信号。

Clinical study of the negative press during puncture in lying-in woman with epidural anesthesia

Objective The effects of gestation on the negative press during puncture in lying-in woman with epidural anesthesia is investigated.Methods A LW group and a NG group,comprising 50 lyirig-in women and no gestation woman respectively,were observed and compared in the clinical study.The members were selected randomly from female patients with epidural anesthesia.Epidural puncture was performed with 18#needle at K2-3.Negative press is tested and the results tire divided positive(+),negative(-),and failure(±).Results The failure records of group LW were markedly higher(P＜0.05)than that of group NG.Conclusion The failure records of negative press were markedly higher in lying-in woman with epidural anesthesia.This shows that the applicability of negative press is deficient in lying-in woman as it is one-index.Thus,we should emphasize particularly the fact that negative press is a caution signal in lying-in woman with epidural anesthesia.
