腹腔镜下腹膜外子宫悬吊术加骶韧带缩短术治疗62例子宫脱垂的临床疗效观察

目的：分析对子宫脱垂患者应用腹腔镜下腹膜外子宫悬吊术加骶韧带缩短术治疗的临床效果。方法将2012年1月～2014年12月进行治疗的62例子宫脱垂患者作为研究对象，均应用腹腔镜下腹膜外子宫悬吊术加骶韧带缩短术进行治疗。观察这种手术方案的疗效，并对治疗前后患者的性生活质量指标进行对比。结果62例患者手术均获得成功，子宫悬吊术的手术治疗时间为（21.5±11.4）min、平均出血量为（13.7±6.2）mL，手术期间及手术后未发生严重并发症。对62例患者进行为期（20.6±8.2）个月的随访，没有失访病例，客观治愈率达到100％。手术治疗前患者的性生活质量评分为（23.64±7.37）分、手术后增加到（38.26±9.01）分，提示患者的性生活质量得到改善，对比术前差异结果显著（ P＜0.05）。盆腔器官脱垂定量（ pelvic organ prolapse quantification，POP－Q）结果提示各个指示点位置变化较大，对比治疗前的差异同样显著（ P＜0.05）。结论对子宫脱垂患者应用腹腔镜下腹膜外子宫悬吊术加骶韧带缩短术治疗的损伤较小、患者的恢复速度较快，且显著改善其性生活质量，具有很高的临床应用价值。

The Clinical Curative Efficacy of Extraperitoneal Laparoscopic Uterine Suspension with Sacral Liga-ment Shortening in the Treatment of Uterine Prolapse(with 62 Cases)

Objective to analyze the clinical curative efficacy of extraperitoneal laparoscopic uterine suspension with sacral ligament shortening in the treatment of uterine prolapse.Methods from January 2012 to December 2014, 62 cases with uterine prolapse were treated in General Hospital of the Yangtze River Shipping,they were chosen as re-search objects.All patients were treated by extraperitoneal laparoscopic uterine suspension with sacral ligament shorten-ing.To observe the curative effect of the operation plan, and to compare the quality of sexual life of the patients before and after treatment.Results the operation plan were successful in 62 patients.the duration of operation was (21.5 ± 11.4)min, the average amount of bleeding was (13.7 ±6.2)mL, and no serious complications occurred during the op-eration and after surgery.62 patients were followed up for a period of (20.6 ±8.2) months, and there was no loss of follow-up cases, the objective cure rate was reached 100%.before operation, the sexual life quality score of the pa-tients was (23.64 ±7.37).after surgery increased to (38.26 ±9.01) points, suggesting that the quality of sexual life of patients was improved, the difference was statistically significant ( P<0.05 ).The POP-Q results indicate that each indicator point varies greatly and the differences between before and after treatment also was significantly ( P<0.05).Conclusions The clinical curative efficacy of extraperitoneal laparoscopic uterine suspension with sacral liga-ment shortening in the treatment of uterine prolapse is remarkable;the operation scheme has the advantages of less inju-ry, faster recovery of patients, and significantly improve the quality of sexual life of the patients, which has a high clin-ical value.
