重症急性胰腺炎合并腹腔室隔综合征的诊治体会

目的探讨重症急性胰腺炎合并腹腔室隔综合征的诊断及临床治疗措施。方法回顾分析18例重症急性胰腺炎合并腹腔室隔综合征临床治疗资料。结果腹腔置管或开腹减压引流组14例，死亡4例，3例穿刺引流减压不充分，腹腔室隔综合征（abdominal compartment syndromes，ACS）持续进展，并发多器官功能衰竭综合征（mulitiple organ dysfunction syndrome。MODS）死亡，1例开腹减压延迟关腹术后，低蛋白血症，合并肠瘘、腹腔感染死亡；未引流减压组4例中3例并发MODS死亡。结论重症急性胰腺炎在保守治疗的同时，应注意腹腔室隔综合征的发生，一旦诊断确立，应及时行腹腔减压。

A study on the diagnosis gild treatment of severe acute pancreatitis compleated with abdominal compartment syndromes.

Objective To investigate the diagnosis and clinical treatment of severe acute pancreatitis combined abdominal compartment syndrome. Methods Retrospect and analyse 18 cases of severe acute panereatitis combined abdominal compartment syndrome clinical data. Results abdominal cavity or abdominal decompression tube drainage group（ 14） cases,4 cases died,3 cases of decompression inadequate drainage, abdominal compartment syndrome （ abdominal compartment syndromes, ACS ） continued developing, complicated by multiple organ dysfunction syndrome （ MODS ） death, 1 cases of open decompression died after abdominal customs delays, hypeproteinemia,combined fistula, intra-abdominal infections;the group without decompression drainage, 3 cases in eases complicated by MODS, lost their lives. Conclusion During the conservative treatment of the severe acute pancreatitis,we should be careful that the abdominal compartment syndrome usually occurs, once the diagnosis is established, abdominal decompression must be given in time.
