高血压与胰岛素抵抗关系的临床研究

目的探讨老年原发性高血压患者与高胰岛素血症、胰岛素抵抗(INSR)、高血脂和肥胖相关因素进行分析。方法对无糖尿病病史的老年原发性高血压患者100例,老年对照组50例的空腹血糖及餐后2 h血糖(FBG)、胰岛素(FINS)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白(HDL)、体重指数(BMI)、胰岛素敏感指数(ISI)和脂肪分布指数(WHR)进行对照分析对比。结果高血压组的BMI,WHR,FBG,FINS,TG水平高于对照组,ISI低于对照组。经多元分析,ISI与TG,BMI,WHR呈负相关,与HDL呈正相关。结论老年高血压患者多伴有糖代谢和脂蛋白代谢异常,胰岛素抵抗所产生的高胰岛素血症为其基本的代谢异常,这是造成高血压病并发冠心病发生的重要原因之一。

A Clinical study of the relationship between insulin resistance and hypertension

Objective To analyze the related factors,such as hyperinsulinemia,insulin resistance(INSR),hyperlipidemia and obesity of senile essential hypertension patients.Methods To compare fasting blood glucose,postprandial 2 hour blood glucose(FBG),insulin(FINS),triglyceride(TG),total cholestero(TC),high density lipoprotein(HDL),body mass index(BMI),insulin sensitivity index(ISI) and fat distribution index(WHR) in 100 primary hypertension elderly patients without diabetes and 50 cases as control group.Results BMI,WHR,FBG,FINS,TG levels of hypertension group were higher than those of the control group,while ISI was lower than the control group.By multivariate analysis,ISI and TG,BMI,WHR correlated negatively,positively correlated with HDL.Conclusion In elderly hypertensive patients with glucose metabolism and abnormal lipoprotein metabolism,insulin resistance generated by hyperinsulinemia was its basic metabolism,it was one of the important reasons which caused hypertension complicated with coronary heart disease.
