MSCT诊断颈部淋巴结病变的价值

目的探讨颈部淋巴结肿大的MSCT表现,以提高病变诊断的准确性。方法 48例经手术或穿刺病理证实的颈部淋巴结肿大患者,均行MSCT平扫及增强扫描,分析其CT表现。结果淋巴结结核10例:多为环形强化;淋巴结慢性肉芽肿性炎1例:病灶较大,边界不清晰;淋巴结反应性增生6例:肿大淋巴结孤立存在,病变平扫密度较均匀,增强有强化;巨大淋巴结增生症1例:肿块均呈均质密度伴显著持续性强化;淋巴瘤5例:增强病灶稍有强化,边界不清晰,部分呈融合状;转移瘤25例:淋巴结均匀或不均匀强化。结论MSCT对颈部淋巴结病变的诊断具有重要的价值。

MSCT diagnosis of cervical lymphnode enlargement

Objective To study the MSCT features of cervical lymphnode enlargement so as to improve the diagnosis of cervical lymphnode disease.Methods 48 cases proved surgically and pathologically who were retrospectively analyzed with MSCT findings of lymphnode enlargement.Results 10 cases of lymph node tuberculosis,typical manifestation was loops with nodes and peripheries were intensified;1 case of lymph node chronic granulomatous inflammation: lesions were large in size and with unclearly boundary;6 cases of lymph node reactive hyperplasia: lymph nodes enlarged and isolated existence,homogeneous density and enhancement on postcontrast CT;1 case of castleman disease appeared homogeneous soft-tissue density masses with marked persistent enhancement;5 cases of lymphoma: lesions were enhancement on postcontrast CT and with unclearly boundary;25 cases of lymph nodes metastatic cancer: lesions were homogeneous or unhomogeneous enhancement on postcontrast CT.Conclusion The MSCT has the important value to the diagnosis of cervical lymphnode enlargement.
