肱骨近端锁定钢板治疗肱骨近端粉碎性骨折

目的评价肱骨近端锁定钢板（LPHP）治疗肱骨粉碎性骨折的治疗效果。方法2004年6月～2007年12月应用肱骨近端锁定钢板治疗肱骨近端骨折32例，男14例、女18例，年龄23～85岁，平均57．1±5．8岁。根据Neer分型：二部分骨折6例，三部分骨折21例，四部分骨折5例。术后利用Constant评分评价其治疗效果。结果随诊10个月～3．5年，平均2．1±0．8年，4例失访，随访期末均顺利愈合。1例出现肱骨头部分坏死，但不影响功能；3例出现肩关节半脱位；2例保守治疗缓解；1例出现肩关节撞击征。根据Constant评分为优良率89．2％（25／28）。结论肱骨近端锁定钢板（LPHP）是治疗肱骨近端粉碎性骨折的首选方法之一．可取得较好的临床治疗结果。

The therapy of proximal comminuted humeral fractures with Locking proximal humeral plate

Objective To evaluate the effects of Locking proximal humeral plate for proximal humeral fractures. Methods From June 2004 to Dec 2007, 32 cases, including 14 males and 18 females, average 57.1±5.8 years old （range from 23 to 85 years old）, with proximal humeral fracture were operated with Locking proximal humeral plate. According the Neer criterion, there were 6 cases with two part fracture, 21 three part fracture and 5 four part fracture. The effects with LPHP were evaluated with Constant criterion. Results 28 cases were followed up from 10 months to 3.5 years （average 2.1 ±0.8 years）. All cases got the union of bone fracture8. Among the cases, 3 cases occurred shoulder half dislocation and 2 cases cured after conserved treatment, 1 cases block of shoulder joint and 1 case humeral head avascular necrosis. According to the Constant criterion, the excellent and the good are 89.2%. Conclusions Locking proximal humeral plate are the best internal fixation for proximal humeral fracture.
