同型半胱氨酸、超敏C反应蛋白、纤维蛋白原与冠心病的相关性研究

摘    要:目的探讨冠心病(CHD)患者血浆中的同型半胱氨酸(HCY)、超敏C反应蛋白(hs—CRP)和纤维蛋白原(培)与CHD的关系,为.CHD的诊治提供可靠的依据。方法对拟诊为冠心病的75例患者行冠状动脉造影(CAG)检查其中稳定性心绞痛18例(SAP组),不稳定性心绞痛22例(UAP组),急性心肌梗死20例(AMI组),冠脉造影阴性者(非冠心病)15例(NC组)],并测定其血浆HCY、hs-CRP、fg水平。结果CHD组患者血浆HCY水平与NC组相比显著增高(P〈0.05),AMI、UAP、SAP3组HCY比较,水平依次降低,且差异有统计学意义(P〈0.05);CHD组患者血清hs—CRP水平与NC组相比显著增高(P〈0.05),AMI、UAP组与SAP组相比,hs—CRP含量较高,且差异有统计学意义(P〈0.05),但AMI、UAP2组间差异无统计学意义(P〉0.05);CHD组患者血浆fg水平与NC组相比显著增高(P〈0.05),AMI组与UAP、SAP组比较差异有统计学意义(P〈0.05),但UAP、SAP2组间差异无统计学意义。冠心病患者HCY、hs—CRP、fg水平在冠状动脉单支病变、双支病变、三支病变组依次增高,两两比较差异具有统计学意义,但单支病变组hs-CRP、fg水平与NC组比较无统计学意义。结论血浆HCY、hs—CRP、fg水平与冠状动脉病变程度及病变支数有关。

Abstract:Objective The main objective of this study is to observe the levels of human serum HCY, hs-CRP, fg in patients with CHD, to discuss whether there exists a relation between HCY, hs-CRP, fg and CHD,To provide an useful evidence for precaution and therapy to CHD. Methods 75 patients who likely were coronary heart disease were performed coronary angiography, stable angina pectoris (SAP) 18 cases; unstable angina pectoris (UAP) 22 cases;acute myocardial infraction (AMI) 20 cases, coronary angiography negative (NC) 15 cases,the plasma HCY,hsCRP,fg level of 4 groups were measured. Results To compare with controls, there are higher plasma levels of HCY in patients with CHD(P〈0.05), the plasma level of HCY in AMI,UAP and SAP decreases in turn, there are significant difference between three groups (P〈0.05) ;there are higher serum levels of hs-CRP in patients with CHD to compare with controls(P〈0.05), there are significant difference between AMI, UAP and SAP patienis (P〈0.05), but there is no significant difference between AMI and UAP groups(P〉0.05) ; To compare with controls, there are higher plasma levels of fg in patients with CHD(P〈0.05), there are significant difference between AMI and UAP,SAP groups(P〈0.05),but there is no significant difference between UAP and SAP gropus(P〉0. 05). The plasma HCY,hs-CRP and fg level increases in turn in single vessel disease, two vessel disease and multiple vessel disease, there are significant difference between three groups(P〈0.05),but the level of hs-CRP, fg have no significant difference between single vessel disease and controls(P〉0.05). Conclusion Plasma HCY,hs-CRP and fg level wase correlated to extent of coronary artery culprit lesion and the number of stenosed coronary artery vessel.
