影响胸椎管狭窄症手术治疗效果的因素

目的总结胸椎管狭窄症的临床特点,探讨影响胸椎管狭窄症手术治疗效果的因素。方法回顾总结获随访120例经过手术治疗的胸椎管狭窄症患者的影像学资料、临床表现、手术过程、手术并发症、术后恢复情况等临床资料。随访时间6～64个月,平均28个月。结果功能评定采用改良的Epstein评分标准,优51例,良42例,改善17例,差10例。优良率为77.5%。脑脊液漏24例;术后症状加重12例,其中2周内恢复到术前水平9例。血肿3例。结论由于胸段脊柱的解剖学特点,诸多合并症等许多因素影响胸椎管狭窄症的疗效。术后症状加重者多可在2周内恢复。

Related factors in surgical treatment for thoracic spinal stenosis

Objective To summarize the clinic characteristics of thoracic spinal stenosis, and investigate related factors in its surgical treatment. Methods We retrospectively reviewed the clinical data of 120 patients who underwent surgery for thoracic spinal stenosis, including radiological findings, clinical symptoms, surgical procedures, complications, and recovery. The fellow-up time ranged form 6 to 64 months with an average of 28 months. Results Function results was categorized by the Epstein scoring as follows: excellent in 51 patients, good in 42, improved is 17, and poor in 10. The rate of excellence and good reached 77.5%. Leakage of cerebrospinal fluid was found in 24 patients. The symptom aggravation was seen immediately after surgery in 12 patients. Nine of them recovered to the preoperative level, and 3 sustained hematoma. Conclusion Due to the anatomical characteristics of thoracic spine, many factors may be related to the outcomes of surgical treatment for thoracic spinal stenosis.
