电子束CT在先天性心脏病的初步应用

摘    要:目的 探讨电子束CT(EBCT)在先天性心脏病(先心病)诊断中的优势和局限性。方法 对82例临床怀疑或诊断为先心病的患者行EBCT心脏单层增强扫描及电影序列扫描,并将结果与超声心动图及手术结果作对照。结果 82例患者中,非紫绀类35例,紫绀类47例,38例经手术或心血管造影确诊,其中EBCT34例诊断正确,误诊4例,EBCT和超声心动图的准确性无显著差异。82例患者中EBCT和超声心动图的符合率为82%。结论 EBCT可以清晰客观地显示先心病病理解剖形态,特别是复杂型先心病,是对造影诊断的有力补充。与超声心动图相比,EBCT在反映主动脉病变,肺动脉发育情况及肺静脉异位引流等方面较超声直观和优越。

Abstract:To explore advantage and limitalion in the diagnosis of congenital heart disease (CHD) by electron beam computed tomography (EBCT) . Methods 82 patients who were suspected or diagnosed as CHD underwent dynamic contrast enhanced EBCT imaging. EBCT imaging was carried out in two steps: single slice mode scanning and multiple slice mode scanning. The EBCT results of 82 cases were correlated with echocardiogram and operation records. Chi - Square test was used for statistical analysis. Results Among 82 patients, there were 35 cases in non - cyanosis and 47 cases in cyanosis. 38 cases were confirmed by surgery or cardiovascular angiography, the remaining cases were confirmed by the multiple imaging exams. And among 38 cases confirmed by Surgery or cardiovoscular angiography, EBCT diagnosis was shown to be correct in 34 cases, misdiagnosed in 4 cases. There was no statistic significance for accuracy in the diagnosis of 38 confirmed CHD between EBCT and echocardiogram. In 82 cases imaging diagnosis, 80% diagnosis of EBCT is consistent with that of echocardiogram's. Conclusion EBCT scanning can demonstrate the pathological morphology of CHD, especially in complex CHD. EBCT is helpful in the study of the heart chamber interior structure, the relationship of great vessel, the atrioventricular connection and the connection between the heart chamber and the great vessel, also in evaluating pulmonary artery development which is complementary or even superior to angiography. It is better than echocardiogram in demonstrating the aortic congenital disease,and anomalous pulmonary veinous drainage.
