老龄高危股骨颈骨折人工髋关节置换疗效分析

摘    要:目的探讨老龄股骨颈骨折人工髋关节治疗的临床疗效。方法治疗48例75岁以上股骨颈骨折。充分术前准备后,经后外侧入路施行人工股骨头置换或全髋置换,术中注意假体位置的准确性及牢固性,防治并发症,术后早期功能锻炼。结果本组18例行人工股骨头置换,30例全髋关节置换,2周患者逐步扶拐下地锻炼。随访9个月~5年,患者无疼痛,关节功能恢复良好,x线假体位置正常,无松动及下沉,效果满意。结论人工股骨头置换或全髋关节置换术治疗老龄股骨颈骨折,具有并发症少、关节功能恢复好、旱期下地、提高生活质量等优点。

Abstract:Objective To explore clinical effect of prosthetic replacement in the treatment of femoral neck fractures in gerontism patients. Methods Forty-eight patients over 75 years old with femoral neck fractures were treated with femoral head replacement through posterior lateral approach of hip or total hip replacement. It was emphasized the exactitude of placement and firmness of the prosthesis, prevention and curing of complications and early mobilization after operation. Results 18 of all cases were treated with femoral head replacement, and 30 cases were treated with total hip replacement. All patients could walk without pain after 2 weeks and followed up for 9 months to5 years. All hips worked well. There was no case of flexure or sinking of the implant. Conclusion Femoral head replacement or total hip replacement is a satisfactory method for the treatment of femoral nect fractures in gerontism patients . It has the less complication, well working hips, earlier weight bearing and better life quality for the patients.
