异环磷酰胺、吡柔比星及顺铂联合治疗晚期非小细胞肺癌的疗效研究

目的：观察异环磷酰胺（IFO）、吡柔比星（THP）和顺铂（DDP）联合治疗非小细胞肺癌（NSCLC）的临床疗效和毒副反应。方法：用IFO1．2g／m^2（第1～3d）、THP45mg／m^2（第1d）、DDP30mg／m^2（第1～3d）的静脉化疗方案，辅以美斯纳、恩丹西酮、集落刺激因子等，治疗晚期NSCLC2个疗程以上者53例，按实体瘤疗效及化疗毒副作用WHO标准进行评价。结果：总有效率52．8％。其中，完全缓解率（CR）为5．7％，部分缓解率（PR）为47．1％；各期有效率为：Ⅲa期84．6％，Ⅲb期62．5％，Ⅳ期29．2％；各病理类型有效率为：鳞癌56．0％，腺癌54．5％，腺鳞癌40．0％；初治和复治的有效率为：初治者65．8％，复治者46．7％；主要毒副作用发生率：白细胞减少81．1％（其中Ⅲ～Ⅳ度为9．4％），胃肠反应84．9％（其中Ⅲ～Ⅳ度为9．4％），心电图异常5．7％，脱发13．2％（均为Ⅰ～Ⅱ度）。结论：IFO联合THP和DDP治疗NSCLC疗效确切，毒副作用可以耐受，该方案可作为目前治疗NSCLC较好的化疗方案之一。
