动态血压监测在老年原发性高血压并左室肥厚临床中的应用

目的：探讨动态血压在老年原发性高血压（EH）并左室肥厚临床观察中的应用。方法：用24小时动态血压、超声心动图观察了51例老年EH与46例非老年EH患的血压动态变化及左室结构改变。结果：单纯收缩期高血压及血压昼夜节律消失在老年EH组多见，尤其是血压昼夜节律消失较非老年EH并发左室肥厚多见。结论：老年EH多有血压昼夜节律消失并与并发左室肥厚相关，多有单纯收缩期高血压。动态血压监测较偶测血压能更好地预测高血压病的靶器官损害。

The Clinical Application of Ambulatory Blood Pressure Monitoring in Elderly Essential Hypertension with Left Ventricular Hypertrophy

Objective: To study the application of ambulatory blood pressure monitoring (ABPM) in the clinical observation in elderly essential hypertension (EH) with left ventricular hypertrophy (LVH). Method: The blood pressure ambulatory changes and left ventricular structure changes of 51 elderly EH and 46 non-elderly EH patients were measured with 24 hour ABPM and echocardiogram. Results: Isolated systolic hypertension (ISH) and disappearance of blood pressure rhythms (BPRD) were common in elderly EH. LVH was more often in elderly EH than in non-elderly EH, especially in elderly EH with BPRD. Conclusion: BPRD is common in elderly EH, which is related to LVH. ISH is often in elderly EH. Ambulatory blood pressure monitoring could predict target organ damage better than casual blood pressure.
