急性阑尾周围脓肿的手术时机及处理对策

目的 探讨急性阑尾周围脓肿的手术时机及处理对策.方法 对1992年1月至2006年10月收治的行I期手术治疗的181例急性阑尾周围脓肿患者的病例资料进行分析.结果 本组无围手术期死亡.发生切口感染27例,腹壁窦道形成6例,盲肠瘘5例,腹腔残余脓肿3例.平均住院天数16.7 d.结论 急性阑尾周围脓肿不是I期手术治疗的禁忌证,积极采取合适的手术方式能及早控制炎症、缩短病程.但必须严格掌握适应证,不可盲目手术.

The optimal operation time and treatment strategy of acute periappendicular abscess

Objective To investigate the optimal operation time and treatment strategy of acute periappendicular abscess.Methods We analysed the data of 181 patients who underwent primary operation for acute periappendicular abscess from January 1992 to October 2006.Results No perioperative death was observed. There were infections of incisional wound in 27 patients, fistura formation of abdominal wall in 6 patients, cecal fistula in 5 patients, residual intraperitoneal abscess in 3 patients. The average hospital stay was 16.7 days.Conclusion Acute periappendicular abscess is not an contraindication of primary operation. Proper kind of operation can control inflammation and shorten the course of disease. But must have a perfect command of indications.
