胰腺癌52例临床分析

目的探讨胰腺癌在普外科中的早期诊断、治疗方法及预后。方法对我院普外科2005年-2008年期间52例胰腺癌患者进行回顾性分析。结果52例胰腺癌患者男女比例为1：1．08，首发症状以腹痛为主，首发体征多为上腹部压痛．辅助检查中CA199、上腹部CT、上腹部MRI、超声内镜灵敏性较高，其中18例患者合并有Ⅱ型糖尿病。8例患者行根治性胰十二指肠切除术，治愈率50％；36例行姑息性手术，好转率36．2％；采取其他治疗方法患者均于确诊后3年内病故。结论对患者有腹痛症状、上腹压痛要考虑胰腺癌可能，及时行有效的辅助检查可早期诊断，及时手术可提高治愈率和好转率。

Clinical analysis of 52 patients with pancreatic cancer

Objective To study the early diagnosis, treatment and prognosis of pancreatic cancer in general surgery department. Methods Clinical data of 52 patients with pancreatic cancer during 2005 to 2008 were retrospectively analyzed. Results The ratios of male and female was 1：1.08, the primarily symptom and sign were abdominal pain and upper abdominal discomfort. CA199, epigast CT, epigast MRI and ultrasonography （EUS） were helpful in diagnosis of pancreatic cancer. Eighteen patients suffered with type Ⅱ diabetes mellitus. Pancreatoduodenectomy were performed in 8 patients, and the curative rate was 50%; palliative operations were performed in 36 patients, and the improvement rate was 36.2%. Patients with other palliative treatments died within three years after final diagnosis. Conclusion For patients involved with pain, abdominal discomfort, the possibility of pancreatic cancer should be considered and efficient examination should be carried out on them for early diagnosis. Timely operation could increase the cure rate and improvement rate.
