子宫内膜样腺癌组织中PTEN、CDK6蛋白的表达及临床意义

目的探讨PTEN、CDK6蛋白在子宫内膜样腺癌组织中的表达及其临床意义。方法采用免疫组化法检测正常子宫内膜、内膜增生、非典型增生和子宫内膜样腺癌组织中PTEN和CDK6蛋白。结果非典型性增生和子宫内膜样腺癌组中PTEN和CDK6蛋白阳性表达率分别为56.7%、46.7%和40.0%、66.7%,两组中PTEN阳性表达均显著低于正常内膜和子宫内膜增生组（P〈0.001）,而CDK6则显著增高（P〈0.003）,二者在非典型性增生和子宫内膜样腺癌组中的表达均呈显著负相关（r=-0.3845,P〈0.04;r=-0.4725,P〈0.002）。癌组中PTEN阳性表达缺失与组织学分级（P〈0.04）、肌层浸润深度或伴有转移（P〈0.02）及临床分期有关（P〈0.03）。CDK6的阳性表达与肌层浸润（Ρ〈0.05）和临床分期有关（Ρ〈0.03）。肿瘤的复发与PTEN的低表达和CDK6的高表达有关（P〈0.05,P〈0.02）。结论PTEN表达缺失和CDK6的过表达可能涉及了子宫内膜样腺癌的发生、发展过程,二者的联合检测可作为临床预测肿瘤复发的生物学指标。

PTEN and CDK6 Proteins Expression in Endometrioil Carcinoma

Objective To study the expression of PTEN and CDK6 proteins in endometrioil carcinoma and its clinical significance.Methods The expression of PTEN and CDK6 proteins were tested by S-P immunohistochemistry on tissues of normal endometrium（30 cases）,endometrial hyperplasia（30 cases）,atypical hyperplasia（30 cases）and endometrioil carcinoma（45 cases）.Results For atypical hyperplasis and endometrioid carcinoma groups,the positive expression rate of PTEN and CDK6 proteins was 56.7%,46.7%,40.0% and 66.7%,respectively.The positive expression of PTEN was significantly lower （P〈0. 001） and CDK6 was significantly higher （P〈0. 003） than those of normal endometrium and endometrial hyperplasia groups. The expressions of PTEN and CDK6 proteins were negatively correlated in atypical hyperplasia and in endometrioid carcinoma groups （r = - 0. 384 5, P〈0. 04;r = - 0. 472 5, P〈0. 002）. In endometrioid carcinoma group the loss of positive expression of PTEN protein was significantly associated with histological grade （P〈0. 04）, muscles invasion （P〈0. 02） and clinical stage （P〈0. 03）. The positive expression of CDK6 protein was sig- nificantly correlated with muscles invasion （P〈0. 05） and clinical stage （P〈0. 03）. The lower expression of PTEN and the higher expression of CDK6 were significantly related with recurrence of endometrioid carcinoma （P〈0.05 ,t~0.02）. Conclusion The loss expression of PTEN and the over-expression of CDK6 protein are closely related to the generation and development of medometrioid carcinoma. Combined detections of PTEN and CDK6 proteins might be a useful biomarker of clinical prediction of tumor recurrence.
