Herbert螺钉内固定治疗肱骨小头骨折

目的评价Herbert螺钉固定治疗Bryan—MorreyI型及IV型肱骨小头骨折的临床效果。方法2006--2012年应用切开复位Herbert螺钉内固定治疗有移位的肱骨小头骨折7例，按Bryan—Morrey肱骨小头骨折分型：I型5例，IV型2例。结果随访12—24个月，7例患者均获得骨愈合而未出现缺血坏死，肘关节稳定、无痛、活动良好，肘关节活动度130度，旋转功能完全恢复，仅1例患者出现轻度关节退变。结论Herbert螺钉固定治疗肱骨小头I型及IV型骨折时可以提供牢固的内固定，使患者术后早期进行肘关节功能康复，获得稳定活动良好的肘关节功能。

Internal fixation of capitellum fractures with Herbert screws

Objective To evaluate the clinical results of Herbert screw fixation in the treatment of capitellum fracture Byan - Morrey I type and IV type. Methods Seven patients with displaced fractures of the humeral capitellum were treated by open reduction and internal fixation with Herbert screws between 2006 and 2012 yr. All the patients were divided based on Bryan -Money classification into following types： 5 cases in type I , 2 cases in type IV. Results All fractures were healed, and no elbow had instability or weakness were found within the follow -up period ranged from 12 to 18 months. The mean ulnohumeral motion was 130~ ,and all the patients had full forearm rotation. Only one patient had a elbow with de- generative change. Conclusion The Herbert screw can provide strong fixation and stability in the treatment of capitellum fracture. It is an ideal material in treating capitellum fracture, and good to excellent outcomes with functional ulnohumeral motion can be achieved following early exercise.
