尺骨鹰嘴截骨Hobert螺钉固定治疗肱骨远端B3型骨折

目的 探讨尺骨鹰嘴截骨Hobert螺钉固定治疗肱骨远端B3型骨折的手术方法及疗效.方法 16例肱骨远端B3型骨折患者均采用尺骨鹰嘴截骨的方法显露,Hobert螺钉固定骨折,尺骨鹰嘴截骨复位后张力带固定.对手术入路、内固定的使用、术后并发症及手术疗效进行评价.结果 16例均获随访,随访时间6～17个月.骨折全部骨性愈合,愈合时间8～13周.肘关节平均活动范围5°（伸）～143°（屈）.参照Cassebaum标准：优10例,良4例,可2例,差0例,优良率87.5％.结论 尺骨鹰嘴截骨Hobert螺钉、张力带固定治疗肱骨远端B3型骨折暴露充分,固定可靠,适合早期功能锻炼,可获得良好的关节功能,是一种较为理想的治疗方法.

Treatment of distal B3 type humerus fractures with olecranon osteotomy by Hobert screw and tension-band fixation

Objective To investigate the effect of distal humeral fractures treated with Hobert screw and tension-band.Methods 16 patients with distal humerus fractures were treated by Hobert screw and tension-band.The outcomes,the approach and the methods of the fixation were evaluated.All fractures got union.The healing time was 8～13 weeks.The average motion of elbow was 5° （extension）～ 143° （flexion）.Results 13 cases were followed up for 6～17 months.Based on Cassebaum score：10 cases had excellent results,4 good,2 fair,and the excellent and good rate was 87.5％.Conclusion Hobert screw and tension-band fixation provides an anatomic reduction and stable fixation and less complications.Combination of early functional exercise provides the good recovery of joint function.
